Long Term Condition Annual Review

A long-term condition is an illness that cannot be cured but can usually be controlled with medicines or other treatments. Examples of long-term conditions include diabetes, arthritis, high blood pressure, epilepsy, asthma and some mental health conditions. Living with a long term condition can have an effect on your role within the family, your job, your accommodation, your education and your finances therefore it is important that you regularly monitor your condition by attending your annual health review.

As part of our commitment to high quality care, and our extended role under the GP contract, the practice invites patients in for an annual review of their condition and medication, this will usually be around the month of your birthday . An invitation will be sent via text message to invite you to arrange your review therefore it is important that we have your most up to date mobile number on your record. Please contact the surgery on 01434 320077 to update this. If you do not have a mobile a letter will be sent in the post to your home address.

While we encourage patients to attend these reviews, if you can’t, or don’t want to attend, we would be grateful if you could notify us so that we do not send you any more reminders until the following year.

Family Planning Service

We provide a full family planning service in house including general contraceptive advice and the fitting and removal of coils and contraceptive implants. Contraceptive advice is available within any of our GP sessions, this appointment will allow you to discuss and look at options available to you and consider which is the best choice for you. After an initial consultation you decide the coil or implant is the contraceptive of your choosing we hold sessions in the surgery with Dr Robertson in allocated family planning clinics which you will then be booked on to for the procedure.

If you are pregnant and would like counselling or advice around abortion you can contact BPAS (British Pregnancy Advisory Service) who will support you to help you decide if abortionadoption or parenting is right for you.

Pregnancy and Midwifery Services

As soon as you find out you are pregnant you are able to book for maternity care directly with Northumbria Healthcare Maternity Services instead of contacting your GP.

Scanning the QR code opposite will take the you to the Northumbria Maternity webpage. Here you will find vital pregnancy information and advice as well as the self referral form to complete.

Northumbria Maternity recommends that you do this as soon as possible so that they can arrange your 1st booking appointment for between 8 – 10 weeks. This also helps them offer screening tests for you and your baby at the correct stage in your pregnancy.

Once you have submitted the registration form, a member of Northumbria community midwifery team will contact you by telephone to arrange your booking appointment. They will also give you access to your electronic hand-held records called Badgernotes.
